Important Information From The Clinic

Clinic Information

Dear Parents,

In order for me to help your child with their individual medical needs, I would like for you to take a moment to review and fill out the appropriate forms so that I may be prepared to address their health concerns. These forms, called Action Plans, will need to be filled out if your child has been diagnosed by a physician with any one of the following medical issues (click on the "Forms and Releases" tab to access the form):

  • Allergies (food, seasonal, chemical, etc.)
  • Asthma
  • Bleeding Disorder
  • Cancer
  • Cardiovascular Issues
  • Cystic Fibrosis
  • Diabetes
  • Inflammatory Bowel Disease
  • Migraines
  • Seizures
  • Sickle Cell
  • Vagal Nerve Stimulator
  • V-P Shunt

You may fill out the action plan and email it to me or you may print it out and send it with your child on the first day of school.

If your child needs medication to be kept at school, please fill out the Administration of Medication Request form in addition to the action plan. Review the Procedures for Administering Medication at School under the Forms and Releases tab above. It has important information about transporting and administering your child's medication.

If your child needs an Epi-Pen at school, please fill out the Administration of Medication Request form and the Hypodermic Injection consent form. The Hypodermic consent form will need to be signed in the presence of a notary.

If you have any questions, you may email me at prima_simmons@gwinnett.k12.ga.us or call me in the clinic at 770-271-4890 starting the first day of school.  Also, please review the Notes from the clinic section for other important clinic information.

Thank you for your time and cooperation in returning these forms to me as quickly as possible.

Prima Simmons
Clinic Worker

Ms. Simmons

I am the clinic worker and love the students and staff of Harmony! My job is very interesting and sometimes entertaining. Days in the clinic are action packed and pass very quickly.

I am a graduate of East Tennessee State University with a degree in Dental Hygiene. My husband, Mike, works for Delta Airlines. I have 3 children. My son, Alex, and one of my daughters, Emily, are students at UGA.  My youngest daughter, AnnaGrace, is a student at Mill Creek High School. I can't think of any better way to spend time (away from Harmony) than with my family!
